Quick add in Personal Vocabulary not working in Dev

The modal appears and you can type stuff in, but when you click submit, the modal does not go away. Even if you close it manually, the word is not added.

This is not quite!

On the Personal Vocabulary & the Dashboard pages, Quick Add Vocab looks like it's going add the Headword, aurum, aurī, n. for aurum, but it just adds aurum under both lemma and headword. See image below.

What I think it is doing is taking the headword from the textbox that the user is typing. I think that would be okay if it one of the radio buttons offered in the quick search isn't selected. If it is, then it should take the headword from that. Also, it should be possible to not select one of those radio buttons, but the top one is selected by default.
