Azure Monitor Exporter

Problem: OpenCensus Node currently does not support Azure Monitor via an exporter.

Solution: Add an exporter to enable exporting of stats and traces to an Azure Monitor account.

Additional context
I am part of a senior design group at NC State University, sponsored by Microsoft. Part of the project they gave us was to add an exporter for Azure Monitor to this project.
