MSSQL - CROSS APPLY implementation

MSSQL syntax has the concept of apply operators which are currently not supported.
I've been trying to figure where/how to implement these as a Function but have been unsuccessful, so opening this.

I would be happy to make a PR back to add it back upstream or as an example to the docs once I know how to actually implement.
