Improve ESM compatibility

This is an umbrella issue to track compatibility improvements for nuxt and across ecosystems with esm modules as they are being widely used.

• Avoid depending on require.main
  • import.meta.main nodejs/node#49440
  • [esm] avoid depending on require.main #6718
  • Not compatible with esm usage telemetry#49
• Issues with externals plugin
  • Error: "Must use import to load ES Module", but I'm already using import! #9223 (comment)
• Rollup plugin to guarantee isomorphic builds without concerning module authors (with siroc and unbuild)
  • cjs <> esm transforms unjs/unbuild#7
• Improvove isomorphic transforms in jiti
• Webpack4 .mjs handling
  • fix(webpack): use javascript/auto for js rule #9180
