filer information form

There are some items on the filer information form that look like they have been setup to reflect the state app, that I'd like to propose we consider changing.

Proposals are:

1. Rename: "Ms/mrs/mr" title to "Title" or "Prefix" (or something).
2. Suggest ordering: Title, First, Middle, Nickname, Last, Suffix
