At this time, the ordering of fields in the contribution form is:

Rec type
Form type
Contributor type
Name title, first last suffix
Address, Address2, City, State, Zip
Is out of state pac, Out of state pac
Date
Amount
In kind description
Employer, Occupation

Suggest something like:

Rec type -> rename "Contribution type"
Form type-> remove (calculate automatically)
Contributor type
Date
Amount
In kind description
Name title, first last suffix
Address, Address2, City, State, Zip
Is out of state pac, Out of state pac -> "Is out of state PAC?", "Out
of state PAC id"
Employer, Occupation
