Render note on referenced attributes by default

Currently note is only rendered when attribute is defined in the corresponding group (being rendered) or if (full) mode is used.

It results in:

• notes added on the referenced attributes when they are appliable on the definition
• (full) rendering mode on the tables, but then all the extended attributes are rendered which is not always desirable. so tags are used to prevent it. E.g. here https://github.com/open-telemetry/semantic-conventions/blob/80a2d1a8f70c96c51e541827c7548ba42bb77876/docs/database/mssql.md?plain=1#L17 and in many other db/messaging semconv extensions

It complicates things and I see no good reason to not render note on referenced attributes.
