Prepend current testnet network name to token name and page titles #514

Whilst it only shows the accounts associated with the network the user has connected to, and whilst on most pages the user just needs to look in the bottom left corner to see what network they're connected to, I still think there's an opportunity to be more specific and provide without it involving too much work.

On the Account page, if I'm connected to a Testnet (i.e. Kovan), it shows my balance under the token name "Ether", and using the token denomination "ETH" for the value. And on the send token tx page and confirm tx pages it displays the same thing, with the transaction heading "Send Ether".
If I switch to the Mainnet (i.e. Foundation), it shows the same thing.

If the user is connected to Kovan for example, then I think the token name should be "Kovan Ether", and on the send token tx page and confirm tx pages the title should be "Send Kovan Ether". What do you think?

I think this change would be adequate, I don't think it's necessary to also change the token denomination.

I think it's a good idea so long as users know what foundation/kovan mean, otherwise they might get confused ("send foundation ether?" = send ether to the foundation?)
