Multiple organization addresses

In the Google Forms-based implementation of the Survey of the International Cooperative Digital Ecosystem 2020 PlatformX has entered "NA" for all address-related fields except country and there they've entered "US & Philippines".

The data model is not set up to handle multiple contact addresses and this seems non-trivial to address.

@erictheise going forward, using a drop-down list for country selection should help with this, do you agree? But there will likely still be cases where entries have more than one location or no specific physical location, so we need to figure out best solution for those
