Creating forms is not possible on GitHub, so it can't be implemented in Probot. You can probably leave a feature request here though: GitHub Community.

Thank you @AaronDewes. Is there any way I can track reactions on a comment, I am trying to subscribe to issue_comments.edited webhook but unfortunately it's not triggered when a reaction is added.

It looks like that is not possible on GitHub either. You could either leave a feature request or write a script that checks for reactions using the API regularly: https://docs.github.com/en/rest/reactions?apiVersion=2022-11-28#list-reactions-for-an-issue-comment. But a Webhook would be more convenient.

Yes, exactly. You can probably also use setInterval() in JS.
