Sometimes RB seems to bail out of using the merge template, with a console log "[refined-bitbucket] User doesn't have permissions to merge this pull request. Skipping "mergeCommitMessage" feature."

This happens when merging PRs by someone else (e.g. when they're on holiday or otherwise unavailable), or merging a PR where not all the checks pass (e.g. when a team agrees that the checks aren't useful for certain kinds of PRs, as I have experienced for automated upgrade PRs that don't require approvals).

Steps to reproduce:
Someone else authors a PR, all checks pass, I attempt to merge

Observed behaviour:
Default BitBucket merge template is used

Desired behaviour:
Merge uses merge template in repository

This is happening for me as well. It makes this feature unusable for me. It seems like it wouldn't hurt to enable this functionality even when all checks haven't passed -- it's not an 'expensive' operation to run.
