docs: Document update behavior for gems without version constraints #11644

Changes:

Document behavior of Ruby Gems only being updated if they have a version constraint specified in the Gemfile.

Context:

#8673.

I'm happy with the formatting of the docs.

I'm not qualified to judge if the docs are technically accurate, so I'll leave that to somebody else.
