add ISSUE_TEMPLATE, move PR template

PR Description

Adding an issue template to reduce the number issues that would recieve better answers on Stack Overflow / gitter. Also should dramatically improve the quality of the issues filed.

Template was borrowed heavily from Babel's, thanks babel!

Merging so that we can get something up to reduce the number of support issues that come in through github (that would do better on Stack Overflow, gitter, etc). Open to any changes to iterate on this going forward.
