This PR will...

Apply and clamp EXT-X-START: TIME-OFFSET according to spec with regards to live window

Why is this Pull Request needed?

The sliding window start was not accounted for TIME-OFFSET is a positive value. This resulted in startPosition being set to TIME-OFFSET when resetting the start position after a live sliding window playlist refresh.
