How to send notifications to discord

I'd like to have the autoheal image send notifications to discord, however im not to sure on how, as there doesn't appear to be any documentation on this subject.
I was able to get the webhook url from discord, however what do I do then? whenver I run the container (and one goes unhealthy) it gives me the following output in the terminal :

How do I actually send a message?
