It seems that the popup appears at a fixed offset from where the cursor is pointing, so you can't see it properly when using a large cursor. To fix this, either make the offset the size of the cursor, or make it so that the bottom right corner of the popup is where the cursor is pointing.

This is bug with the Operating System and reproducible with multiple applications. There's also not much we can do when the OS doesn't have functions returning an accurate cursor size and the tooltips control their own window location.
I wanted to write exactly the same issue not long ago. I use large cursors as well, and this problem has been bugging me for a long time.
But a few days ago I decided just to switch cursor. Now I use big thin cross as a main cursor. It was awkward at first, but once you get used to this new look, you'll never wish to switch back.
Also, in a browser there is another especially frequent type of cursor, a hand with a pointy finger. You'll probably want to replace it as well. Rest of the standart cursor set is not so annoying.
