DNS over HTTPS support, Bug Fixes

The largest change in this release is DNS over HTTPS (doh) support (thanks @jwijenbergh).
DNS over TLS was renamed from "tcptls" to "dot" in the command line options and documentation.
There were also several bug fixes covering the rate limiter, the build, and the docker image.
