v1.0.0

There are almost no differences with 0.6.X.
So many bugs fixed, it's working in production for 5+ months and looks stable enough for 1.X.X.

Also this branch will support fastify 1.X.X. When fastify 2.X.X will be released there will be also major version bump due to submodule updates.
