Option --all works inconsistently, now it stopped working, that's why coverage has increased (istanbuljs/nyc#849). Even when it worked, it was implemented quite terribly via glob.sync(this.cwd) instead of using include paths (istanbuljs/nyc#782), thus walking through millions of files in node_modules et al.
