  1. feat: Add support for --exclude-node-modules to subcommands. 

    This adds support for the `--exclude-node-modules` option/setting to
instrument, check-coverage and report sub-commands.

Add testing to verify that `--exclude-node-modules=false` is honored for
all commands.
