create template to guide CSV design for exports

approximately 1 month ago, it was suggested that a template CSV or spreadsheet file be made so that everyone can iterate on the contents and format of exports from the curator-portal. Other work has had priority, but this should be done to avoid having to re-work back-end design decisions.
